Holiday Checklist: Does Your Loved One Need a Caregiver?

Clinical Care & Home Health Aide

For older adults who have complex medical conditions, extra care is often needed. We meet these needs by offering home health aide caregivers and clinical support. We send a clinician to your home that can review your care plan and recommend additional clinical resources on an as-needed or recurring basis.

Whether you’re looking for the convenience of a home visit by a doctor, periodic home safety checks, or remote monitoring, it’s important to know our Clinical Team works seamlessly with the CareTeam you already know and trust.

  • Our Clinical Care and Home Health Aide services include:

    • Personal Care Closed Open

Help with bathing, grooming, dressing, toileting, cleaning, and Activities of Better Living™ Learn more

    • Home Safety and Falls Prevention Assessment Closed Open

An environmental scan of the home to determine if adaptive equipment – grab bars, hand-held showers and transfer aids – is needed to keep you safe. Learn more

    • HouseCalls and Telemedicine Closed Open

A physician conducts an initial wellness visit and is available to speak to you by phone to review care, discuss medications, and perform a range of services to provide clinical oversight when needed, or on a recurring basis.

    • Medication Management Closed Open

We coordinate medication pickup, organization, and reminders to ensure adherence to your medication regimen.

    • Tablet-based Monitoring Closed Open

We equip your home health aide with tablet-based technology to record important data about your loved one’s health, well being, and daily activities. Our CareTeams use this data to keep families informed and connected, stay ahead of health issues and in some cases, prevent hospitalization.

Thanks again…Everyone has been awesome. Really, I don’t know how you all do what you do. It’s difficult to support seniors with ever-changing needs and depression/anxiety which can make things feel like a yo-yo. You are all such a gift in our lives. And, the consistency you offer makes all of the difference. Plus, how everyone communicates with each other.
